If your ball player is under the age of 18 years of age, we ask that you complete the appropriate medical form. If your are signing up your child for baseball instruction, please complete the Baseball Medical Release Form. If however, you are enrolling your child in PSBT’s Softball instruction program, for Softball, please complete the Softball Medical Release Form

It is important that we have this document on file, in the event medical attention is required. Although I run a safe practice, and will treat your child with the same care and attention as I do my own. As a parent, I know accidents and/or emergency situations can occur. If they do, I want to feel certain that your child will receive proper medical attention. Therefore, please take a moment to carefully complete the form that pertains to the lessons you are signing your child up to receive.
